Using a First Health PPO in-network provider still helps by simplifying billing and reducing the chance of balance billing above the negotiated rate.
  • Pharmacies are Out-of-Network: No pharmacies are listed as in-network for First Health PPO. All pharmacy claims are treated as out-of-network, and the applicable deductible will apply.
  • Dental Providers are Out-of-Network: All dental providers are considered out-of-network. The out-of-network deductible applies to eligible dental claims for injury to sound natural teeth.

If you get sick or injured - including a sudden acute onset of a stable pre-existing condition or COVID-19 - visit a First Health PPO in-network provider where possible for smoother, direct billing.

For inpatient hospitalization, surgery, or a surgical procedure, call Trawick's plan administrator to pre-certify. Skipping this step cuts eligible benefits by 50%.

Economy through Platinum tiers have a $0 deductible; Diamond and Diamond Plus (ages 70-89) apply a $100 or $200 deductible.
