If you are charged more than what they pay, you will have to pay the difference (after your deductible). With a comprehensive plans, they pay more fully after you pay your deductible (which you choose at the time of purchase) and then, sometimes a coinsurance (usually 20% of the first $5000 in expenses).
With either type of plan you can use any doctor or hospital, but using the plan's PPO network (preferred provider organization) can help reduce costs (reduce or eliminate coinsurance) and help your money go further (with negotiated pricing). All you need to show is your ID card and the medical provider can call to check your benefits and arrange payment. However, if they are not willing to make the call (they may say "We do not take that insurance"), then you can still use them, but you will have to pay up front for all medical treatment and submit a claim to get reimbursed from the insurance company for what you overpaid.
