Students and scholars on F or J visas have the best coverage for maternity coverage. However, the benefits have been reduced a few years ago such that the best coverage is 80% coverage of costs, so please be aware that you will be responsible for a portion of all costs. Also note that unless your wife is the J1 visa holder YOU will have to be the primary on the policy and she (with J2) will be secondary. Also note that prices are DIFFERENT for the primary and the spouse, so when you quote the prices, quote them for two people at the same time since the cost will be MUCH HIGHER for your wife's coverage.

There are five policies that cover maternity at an 80% coverage without a set limit on benefits. I will list them in order of their approximate cost. Note that conception has to occur after the start of the plan and most plans ask that you let them know at least three months before your delivery date.

Study USA Pref 300 and Pref 500 both cover maternity at a 80/70% (in network/out of network). Conception has to occur after the start of this plan. This plan will be about $133 for the Elite plan.To get details for Study USA, , please visit the below links

Liaison Student plans Choice and Plus have a $10k limit on maternity benefits at 80/60 coverage respectively. The Choice plan will cost about $151 per month.To view all information on the insurance, please visit the below links:
Student Secure Secure Elite, Seletc and Budget plans also cover at 80/70% to the maximum of the plan. The Select plan can be purchased with a $300k maximum, Budget plan can be purchased with a $250k maximum and the Elite can be purchased with $500k.The Elite plan will cost $375 per month, Select about $220 and Budget about $95. Student Health Advantage Platinum has a $5000 maximum limit to maternity and newborn care coverage. It has a $1 million maximum and a $35 deductible and covers at 80/60%. The plan also covers pre-existing conditions with a 6 month waiting period. This plan will cost about $314 per month.

The insurance plan must cover the following: Medical benefits of at least USD 100000 per accident or illness; remains repatriation coverage of at least USD 25000 per person covered, coverage for expenses associated with medical evacuation to the international student’s home country of at least 50,000 per person covered; and Deductibles that do not exceed 500 per accident or illness. The Patriot Exchange plan is a very economical plan that can be used to meet F2 visa requirements and for international students. The plan can be purchased for the F2 visa holder as long as the F1 visa holder already has coverage from their institution. The deductible can be $500 or less with no further costs for the insured (most plans have coinsurance which still must be paid after the deductible). Also, if the institution has a health center, you can pay only a $5 copay for each visit for additional benefit and savings. You can purchase this plan with a sports rider to cover such activities such as downhill skiing. All these benefits make it a popular and excellent choice. This is the only student plan available for F2 while F1 is already on a plan. Copay is the amount that you pay for each doctor visit. If you have chosen the plan with a $0 deductible, then you need not pay the copay.
The maximum coverage that the plan will provide will be not more than what you have selected as policy maximum on your plan.
Let's say you have bought a plan with $50 deductible and a policy maximum of $50,000.

When you visit a doctor for some illness and let's say you incur a cost of $500 for the medicine and the check up. For the first time visit and medical expense you incur, you will be paying the $50 deductible and then you will pay $15 as co pay. The insurance company will be paying $435.
This plan says that the insurance company will pay 100% of co-insurance.
So, you do not have to worry about that.
