I need insurance for my wife who is coming to US on H4. Can you please explain the coinsurance part of the plan which costs me $5,239 and coverage of $50,000 . Can you please let me know, how the coinsurance works for the plan?

This is how co insurance works for this plan.
$50,000 is the policy maximum. This is the maximum that you have chosen which the plan pays in case of any kind of emergency such as emergency evacuation etc.
Now coming to how the plan pays in case of individual expense:
If you incur medical expense of lets say $5,239 and you have selected a deductible of $100. Then here is how the expense is calculated.
5239 -100(amount that you need to pay) = 5139. Now the co insurance is applicable for the first $5000 only in case you got to a network outside the PPO. In that case, you will pay the 20% of $5000 which is $1000 and the rest will be paid by insurance company. So out of 5239 you will be paying 100(deductible) + 1000 = 1100 and the rest is paid by insurance company.
