How does the claims process work with direct billing for visitor medical insurance?

Do the visitor medical insurance plans pay first and reimburse later? Or should I go through the network and pay only the deductible?

support_agentDear Roger,

Thank you for contacting us at American Visitor Insurance for your US visitor health insurance needs! With all the plans we sell, there is a possibility that you can get direct billing for any medical expenses whether incurred in the US or abroad. In the US, it is primarily determined by the provider. If the doctor's office is not willing to make the phone call and arrange payment, then you will have to pay up front and submit a claim for reimbursement.I know from working with the insurance companies that they are quite willing to work with providers.

Note that for coverage in the US, most plans have a deductible and a coinsurance that must be paid (deductible is chosen at time of plan purchase). The coinsurance is usually 10% or 20% of the first $5000 in expenses. International plans outside US and Canada do not charge coinsurance after the deductible (it was not clear where the traveller was heading from your email).

Please let us know if we can help you in any further.

How is the claims process handled with direct billing or reimbursement for Visitor Secure Plan D?

For your mentioned plan of Visitor Secure PlanD, how is the claim settlement done? Is it something that the insurance company pay directly to hospitals or the patient needs to claim retrospectively?
Also, please explain what $50,000 means in pre-exisiting illness coverage.

support_agentDear Udit Dalal,

Thank you for contacting us at American Visitor Insurance for your visitor medical insuranceneeds!

The claim payment is totally dependent on the doctor/provider that you go to. Normally if you to a doctor facility within the PPO network of the insurance company, then they might be willing to call the insurance company directly and do the direct billing. If not, you will have to pay first and submit the scanned bills to the insurance company and they will reimburse it. Both of them are possible. But I would like to make it clear that claims and bill payment is not in our control.

Regarding your second question, pre existing coverage is available in Fixed plans like Visitor Secure up to the age of 70. By Pre existing condition coverage, the plan covers only the acute onset or the sudden recurrence but nothing apart from this. Again the report from the doctor should mention this for the insurance company to cover it. $50,000 coverage is the maximum amount that you can use on your policy for pre existing condition coverage if you buy plan with a policy maximum of $50,000.

Please let us know if we can help you any further.

How are insurance claims handled for prescription drugs at a pharmacy?

Please tell me what I need to show at the pharmacy, because I went once with my actual insurance and they tell me my insurance didn't cover medication!
In the other hand, I know dental is out, this is why I ask you for a dental complement for me and my daughter, can you send me some options to buy it?
commentUdit Miriam
support_agentDear Miriam,

When a provider says "We don't take your insurance." What they mean is:

We don't want to call and arrange payment with your insurance company.

All eligible medical expenses (including prescribed medications) will be covered by your insurance according to the terms of the insurance plan. All you need to do, if the provider will not make the call, is fill out a claims form (after you pay for whatever it is you need; this will be the same for medication, doctor visit, etc.) in order to get reimbursed.

As far as dental goes, the Atlas plan has a fixed coverage for "sudden pain". So anything after that you will pay for yourself.

Let us know if you have any other questions or issues. I will put a link to the claims form for your convenience. It includes instructions on where to send it.

support_agentDear Samira,

Thanks for contacting us with your Insurance inquiry!

There is no monthly payment option to purchase this plan. You can renew the plan as often as you want. If you purchase at least one month the first time you buy it, the company will waive the renewal fee (only $5). However, you have to remember to extend the policy each month or however long you wish to extend the policy. IMG will send you a reminder email near the end date of your policy that it is available for renewal.
Here is the buy link for this plan again for your reference:

What is direct billing in insurance?

Direct billing is also known as cashless insurance that allows the healthcare provider bills the insurance company directly for medical services instead of the patient paying upfront. This simplifies the process for insured individuals, reducing out-of-pocket expenses at the time of treatment. However, not all providers accept direct billing, and patients may still be responsible for deductibles, co-pays, or uncovered services. Travelers and expatriates should check if their insurance plan includes direct billing with hospitals.

What are the J2 Visa Health Insurance Requirements?

To file a travel medical insurance, claim, submit a completed claim form, a copy of the insured's passport, and supporting documents like medical bills, pharmacy receipts, or doctor's notes.

  • Direct Billing: The provider sends the bill directly to your insurer, but a claim still needs to be filed for approval of eligible expenses.
  • Upfront Payment: Upfront payment requires you to pay medical costs out-of-pocket, then submit receipts and documentation for reimbursement.

How does upfront payment work?

If you paid upfront for medical expenses with a visitor's insurance policy, follow these steps to file a claim for reimbursement:

  • Obtain an itemized bill from the healthcare provider.
  • Retain all receipts and proof of payment.
  • Keep any medical documents of the payment you made upfront
  • Submit the necessary claim form along with your supporting documents to your insurance company.

Once the claim is reviewed and approved, your insurer will reimburse eligible expenses based on the policy's terms.

How long do travel insurance claims take?

The time it takes for a travel insurance claim to be processed depends on the insurer, the nature of the claim, and how quickly you submit the necessary documents. For example simple claims, like trip cancellations or medical reimbursements, may take 1 to 2 weeks, while other cases, such as medical evacuations or lost baggage disputes, can take 30 to 60 days. To speed up the process, ensure all documents (receipts, reports, and forms) are complete and accurate.

What are claims in medical billing?

In medical billing, a claim is a formal request sent by a policy holder to an insurance company for reimbursement of medical services or covered loss. The insurer reviews and validates the claim, once approved issues payment to the insurer or the authorized party on behalf of the insurer.

What are the most common travel insurance claims?

The most common travel insurance claims include medical expenses, trip cancellations, and lost or delayed baggage.

  • Medical Expenses: Unexpected medical conditions can arise when traveling. This includes coverage for hospital stays, doctor visits and emergency medical evacuations. Medical expenses abroad can be high, making this one of the most common claims.
  • Trip Cancellations: Trips can be canceled for various reasons, including illness, family emergencies, or travel advisories. In such cases, travel insurance can reimburse non-refundable trip expenses, helping to ease the financial burden if your plans fall through unexpectedly.
  • Lost or Delayed Baggage: Baggage mishaps, such as lost, stolen, or delayed luggage, are frequent claims for travelers. Insurance can provide compensation for essential items, ensuring that you are not left without necessities while waiting for your bags to arrive.
  • Travel Delays: Flight delays or missed connections due to weather, technical issues, or other factors can result in additional costs such as accommodation, meals, or rebooking fees. Travel insurance can help cover these unexpected expenses, ensuring you don’t bear the full financial impact of travel delays.

Can I demand direct billing from a hospital?

No, you cannot always demand direct billing. It depends on whether the hospital has an agreement with your insurance provider. If direct billing is not available, you'll need to pay upfront and file a claim for reimbursement.

Can hospitals demand payment upfront?

Yes, hospitals can demand payment upfront, especially if the treatment is not urgent or if you don’t have insurance coverage with them. For non-emergency services, hospitals may require payment or a deposit before providing care. If you have insurance, you can file a claim for reimbursement after paying upfront.
