My daughter is US citizen and will be coming to the US to stay there for 9-10 months a year. She is a dual citizen where she is US citizen and NewZealand citizen.
I have a few questions on the INF plans and the Diplomat plan.
(a) Firstly, with the INF plans, it would seem from the plan documents that these are straight fee-for-service plans where she can go to any Doctor/hospital/pharmacist she likes with the corresponding deductibles and plan maximums prescribed. Is this correct? However, I also see on your website that INF has a PPO network, First Health, which has many providers within good distance of where my daughter will be going to College.
Does the INF Standard Plan enable her to use this network, which would obviously keep costs down, while giving her the option to go outside of the network if necessary (but unlikely)? Also, what is the coverage for prescriptions on the INF plan, it is not clear from the plan brochure.
(b) The Diplomat America plan, same questions, it seems there is a PPO network, PPO network that is available and also has lots of providers in her College’s area? Prescription coverage for Diplomat America or no?
Lastly, with both plans I notice they require a passport number upon application. As a dual citizen, do we enter her US or her New Zealand Passport information?

Here are the answers to your questions:
a) Yes, the INF plans have the PPO network like the First Health network and you can get covered under that PPO network with the INF plan. For the prescription coverage, it covers 80% of the network charges up to the overall policy maximum.
his means that if you have a claim expense for instance say $4000, then you can go to any provider who comes under the First health and after the deductible, 80% of the expense is covered by INF. If you selected $500 deductible, then after paying the deductible for the first time expense, out of the rest $3500, 80% will be paid by INF and 20% should be your payment.
This is the co insurance factor. This is for the comprehensive plans like the Elite and Traveler USA. If you happen to go to a provider who does not come in the network, then 60% of the claim expense will be covered by INF.
For the INF fixed plans, like the Premier and the Standard, the brochure will have the specific amount that is covered for the prescription medication.
b) Diplomat also have their PPO network, their PPO network information is here: Diplomat America PPO network.
You will have to call the number given in this pdf to find out more. Regarding the prescription medication, there is coverage for that. Please see here: Diplomat America Plan Summary.

I have a 19 year old son, who resides with me in Spain. DANIEL is American, so he does not need a visa. From August 2019 to July 2020 it is planned that I studied at Manhattan College in NY. The College requires us medical insurance coverage without exclusions, or lack of deadlines for pre-existing diseases for the entire school year (August-2019 / July 2020), because although Daniel is in very good health, is Diabetic from 14 years of age (Melitin type 1).
What would be the most appropriate plan for Daniel's circumstances?

Unfortunately we do not have anything that will fit what you are needing for your son while he is at Manhattan College. However, since he is a US citizen, he can apply for domestic coverage (so called, Obama Care plans) offered by private companies which you can compare from New York's insurance marketplace.
Ordinarily, you have to wait until what is called "open enrollment" periods (starting in November for plans to start the first of the year). However, his arrival into the US (whenever that will be) will constitute a "qualifying event" which will allow him to sign onto a plan mid-year. He will have a limited time in which to sign up, but domestic plans all have to meet the requirements you describe.
The other option is that the College may offer a plan for their students. This should meet all their requirements. Either of these options are not inexpensive, but they will be what he will need during his time at Manhattan College.
The visitor plans we offer (some of which can be purchased by US citizens for short term coverage while in the US will not cover pre-existing conditions and are only for sudden illness or accident (so there are many exclusions). Here is the plan that is available for US citizens coming to the US for short term. Universities sometimes ask for specific benefit coverage. That is something that you might want to check with your university.
Regarding how these policies work:
You can buy them online and you will receive a confirmation email with the ID card and policy details attached. You can use them for your future use.
The deductible is the initial amount that you will be paying in case you incur claim expense after which the company will pay the remaining expense.
There is a 20% co insurance amount that is applicable after the deductible amount for the first $5000 of the expense after which the insurance company will pay 100%.
The other option is Patriot America insurance from International Medical Group. The policy maximum range from $50,000 to $1 million. The traveler must have resided outside US continuously for the past 6 months and must have a current health plan in force.
