The plan pays 90% of first $1,000 then 100% up to the policy maximum after the deductible within the provider network in the US, and the plan pays 80% up to the policy maximum in outside the network in the US. Outside the US, the plan covers 100% up to the policy maximum after the deductible. This plan offers coverage for minimum of 1 month up to 12 months and it can be renewable up to 48 months. It offers $5 co-pay per visit to Student Health Center. All other J categories are not subjected to the individual mandate for two years. Since international students are not subject to the mandate, they are not required to purchase a plan that meets PPACA requirements and can purchase the Patriot Exchange program.
