New US Health Requirements While Applying for US Visas

The US State Department has directed consular officers across different embassies to consider a visa applicant’s health condition in determining whether the applicant will become a ‘public charge’ of the United States, and therefore, deemed ineligible for a visa.

Conditions such as diabetes, asthma, high blood pressure, cardiovascular disease, mental health issues, or obesity may be weighed more heavily during the visa assessment process.

To minimize the risk of visa rejection and demonstrate preparedness, travelers should purchase comprehensive travel medical insurance before applying for their US visa. Having valid travel medical insurance coverage shows that the applicant can manage any health expenses without becoming a ‘public charge’ or a financial burden during their stay in the U.S.

How Does Coverage for Pre-Existing Conditions Work?

  • Definition of a pre-existing condition – what medical conditions are considered pre-existing.
  • Look-back period – the time frame prior to travel during which a condition is considered pre-existing.
  • Type of coverage – whether the plan covers only acute onset (sudden flare-ups) or full coverage (ongoing treatment).
  • Coverage limits – the maximum amount the policy will pay for pre-existing conditions.
Whether a travel health insurance plan covers your pre-existing conditions depends on all these factors.

Acute onset vs. Chronic Conditions

Type Definition Examples
Acute Onset of a Pre-Existing Condition A sudden and unexpected onset of a pre-existing condition that occurs after coverage starts and requires immediate medical attention. Heart attack, sudden asthma attack, acute appendicitis
Chronic Pre-Existing Condition A long-term or ongoing condition that existed prior to coverage and is generally managed over time Diabetes, hypertension, chronic kidney disease

Some visitor insurance plans cover acute onset of certain pre-existing conditions but do not cover chronic management.

How Can You Tell If a Condition Is Considered Pre-Existing for Travel Insurance?

A pre-existing medical condition refers to any health problem you had before your travel insurance started, even if you didn’t know about it.

Insurance companies check your recent medical history (called the look-back period, usually 60 to 180 days) to decide if something counts as pre-existing.

This includes any condition where you:
  • Were diagnosed or treated by a doctor.
  • Were taking medicine for the condition.
  • Had symptoms or tests related to it.
  • Were advised to see a doctor for it.
Example:

If you took medicine for high blood pressure or diabetes a few months before your trip, that will be seen as a pre-existing condition.

Why Should I buy Visitor Insurance for Pre-Existing Conditions ?

There are several compelling reasons for buying Visitors Insurance for Pre-existing conditions. Some of the important reasons are :

  • Coverage for Sudden Medical Emergencies: Visitor insurance that includes acute onset of pre-existing conditions provides protection if a long-term health issue (like diabetes or heart disease) suddenly flares up during your trip. This helps travelers get timely treatment without facing large medical bills.
  • Comprehensive Medical Support: These plans not only help with sudden complications of pre-existing conditions but also cover new illnesses, injuries, and doctor or hospital visits that may occur while you’re in the U.S.
  • Protection Against COVID-19 and Other Infections: If you contract COVID-19 or another covered illness while visiting the U.S., your insurance can help pay for the necessary treatment, ensuring your trip isn’t disrupted by unexpected medical costs.
  • Emergency Assistance and Evacuation: If your condition worsens and requires transportation to a better-equipped hospital or your home country, emergency medical evacuation benefits can take care of those arrangements and expenses.
  • Financial Security During Travel: Visitor insurance with pre-existing coverage helps you travel confidently knowing that if your health condition unexpectedly worsens, you won’t face overwhelming out-of-pocket expenses.
  • Ideal for Senior Travelers: This type of coverage is especially valuable for parents or elderly travelers visiting the U.S., providing peace of mind and essential medical protection throughout their stay.

What Are the Types of US Travel Insurance From Brazil?

Fixed Benefit travel insurance

A fixed benefit or scheduled benefits travel insurance plan is the cheapest US travel insurance plan, however this is because the Fixed benefit travel insurance have a fixed limit of coverage for different medical benefits. Fixed benefit international travel medical insurance works well for small ailments but can be inadequate for more serious medical conditions involving hospitalisation.

Comprehensive travel insurance

Comprehensive travel insurance provide full coverage for medical expenses up to the policy medical maximum limit. As opposed to fixed benefit international travel medical insurance there are no coverage limits for each benefit. It is much more expensive when compared to fixed benefit plans but also provides significantly superior coverage especially in case of a serious medical emergency or hospitalisation.

  • Medical Coverage: Ensure the policy covers medical expenses, including hospitalization, doctor visits, and emergency medical treatments. Check if it includes coverage for pre-existing conditions if applicable.
  • Trip Cancellation and Interruption: Look for coverage that reimburses you for non-refundable expenses if your trip is canceled or interrupted due to unforeseen.
  • Baggage and Personal Belongings: Check if the policy covers lost, stolen, or damaged baggage and personal items.

Can I Get US Insurance When I Am Already in the US?

YES! Brazilians can purchase US travel insurance policies from American Visitor insurance to cover the remainder of their stay (as long as they like).

  • Policy Flexibility & Renewability:
    • Note that our policies are generally renewable, so if plans change (as they so often do these days), the policies can be extended (minimum of 5 days and up to 12 months).
    • Policies are available up to $100,000 for persons in their 70s and up to $50,000 for persons in their 80s.
    • You can use our search engine to determine the best prices for you based on age and deductible you'd like.
  • Deductibles & Cost Considerations:
    • The deductible on a comprehensive travel insurance USA policy is generally a one-time payment if there are any medical expenses (doctor visit, ER visit, medications, etc).
    • A higher deductible will give a lower premium cost, so it is a balance of what you feel you can pay out in the unlikely event you will need medical attention, and how much the policy costs you per month and day.
  • Application Process & Address Requirements:
    • When you purchase the US travel insurance policy, you will enter your Brazilian address as your "home" or "residence" address regardless of whether you own a home in the US or not.
    • The "residence" country and the "destination" country CANNOT be the same.
    • Then, you can choose the start date (even if it says trip start date) as the first day you need the policy.
    • And, even though you choose an "Trip end date" you can always extend most policies without issue (just do not wait until they have expired).
  • Pre-Existing Conditions Coverage Details:
    • Unfortunately, insurance coverage for pre-existing conditions is very limited on most US visitor policies for foreign visitors.
    • Some policies will cover "acute onset" of pre-existing conditions which should mean life-threatening emergencies due to pre-existing conditions, but they have all recently excluded chronic and congenital disorders from that coverage.
    • The Safe Travels Elite (a fixed plan) and Safe Travels Comprehensive offers some coverage $25,000 or $15,000 for a cardiac issue.
    • While there will always be some risk involved while traveling and staying in the US, travelers do have travel insurance options for visiting the US.
  • Choosing the Best Plan:

What Is the Best Travel Insurance for Seniors With Pre-Existing Conditions?

  • Pre-Existing Condition Overview : Pre-existing condition is a medical ailment that existed before the travel insurance became effective, and “Acute onset of pre-existing condition” refers to a sudden relapse requiring medical attention within 24 hours.
  • Medical Conditions Considered Pre-Existing: Travelers with pre-existing conditions always find it hard to buy travel health insurance; common conditions include asthma, food & drug allergies, environmental allergies, heart related issues, high blood pressure, kidney disease, cancer, and diabetes.
  • Coverage Limits & Look-Back Rules:
    • Insurance providers have their own terms for pre-existing condition coverage, and most do not cover chronic illnesses like cancer, Parkinson's disease, or amnesia; with pre-existing conditions there is a "look back" period of 60–180 days to determine the claim.
    • Most of the policies do not provide coverage for pre-existing conditions at the age of 70 and more.
  • Senior Coverage Options:
    • Some US insurance providers offer senior citizen travel insurance for pre-existing conditions above the age of 80 with fixed or limited coverage.
    • The Safe Travels USA, Safe Travels USA Cost Saver, and Safe Travels USA Comprehensive offer $1000 coverage for pre-existing conditions without limitation to chronic illness.
  • Choosing the Right Pre-Existing Coverage: If travelers have a medical condition they are concerned about, it is very important for them to buy a policy that includes the illnesses in the coverage and to be aware of the terms of pre-existing condition coverage.

  • Brazilian students should enroll in a suitable health insurance plan to cover medical expenses, as healthcare in the USA can be expensive.
  • Universities often offer their own health insurance plans, which students can choose to enroll in.

  • Optional Practical Training (OPT) allows for temporary employment directly related to their field of study for up to 12 months after graduation.

Brazilian Migration History:
  • Brazilian immigrants in the USA have a long history of migration, primarily driven by economic reasons.
  • The first significant wave of migration occurred in the 1980s and 199s, with travelers from Brazil seeking better economic opportunities and escaping hyperinflation in their home country.
Yo-Yo Migration Pattern:
  • The migration patterns of Brazilian immigrants have changed over time.
  • Initially, many Brazilians planned to stay in the USA for a few years, earn higher wages, and then return to Brazil with their savings.

Recent Immigration Shifts:

Citizenship Status:
  • Brazilians in the USA are less likely to be naturalized U.S. citizens compared to the overall immigrant population, with only 35% having obtained citizenship as of 2019.

  • While the US provides one of the best medical services to its immigrants, it is also very expensive.

Can a US Citizen Living in Brazil Buy Travel Insurance for Trip Cancellation for Visiting USA?

Hello,

I am an American (age 53) married to a Brazilian woman (age 45, who does not have a green card), and we live in Manaus, Brazil. We will be traveling to the US from February 25 to March 12. My wife has gotten both vaccinations plus one booster, and I have gotten the two vaccinations.

I would like to know what types of travel insurance would be available to us given our circumstances, both regarding trip postponement/cancellation and health care.

If you could please let me know, I would appreciate it. Thanks in advance for your help. I look forward to your response.

Thank you,
Scott

Dear Scott,

Unfortunately, only US residents can purchase trip cancellation plans from US companies (this change happened a few years ago). However, you can purchase travel health insurance. As a US citizen, you don't have that many choices, but the choice you do have comes from a solid company (you can get it for your wife as well).

While the plan does not include trip cancellation, it does include trip interruption, baggage loss, and trip delay (and other useful travel benefits).
